Case Studies
Protocol
The information provided by the person filling out the formoline case study questions will be used to gain feedback, and insight into the consumers' experience and opinion of the product. It will not be used for clinical data and SSL, Certmedica or TBWA will not be publishing the results as statistical data. To fit the criteria to take part in the formoline case study, you will need to be at least 21 pounds (a stone and a half) overweight, and you are willing to take the product formoline L112 for up to 4 months.
Due to the high fat binding capacity of formoline L112, all fats and fat-like substances can be bound, including vitamin A or steroids found In oral contraceptives -“the pill”- and in menopausal discomfort treatments. Formline L112 may also impair the availability of fat-soluble (lipophilic) substances. Therefore, avoid associated use of medicines or vitamins for at least 4 hours.

In exchange for the opportunity to participate in the formoline case study and for other good and valuable consideration, I hereby grant to SSL-International plc, Certmedica and TBWA the unqualified right to reproduce, copyright, publish, circulate, or otherwise use my name, image, photograph, likeness, voice and signature produced by SSL International or on their behalf by any other entity or person. Hereinafter. I warrant that I am the owner of the information I provide and I give express permission to the released parties to make alterations as required to meet the requirements for broadcast or dissemination.
I warrant and represent that all statements and information I make, give, or relate to any Released Party or any other entity or person in conjunction with my participation in the Testimonials are truthful and factually accurate.
